School Needs Assesment Survey

Please offer your ideas on editing the HPSA needs assessment survey to be distributed to local schools. We can not know what types of programs to create or how to create them without first knowing what is needed. Thanks for your comments. View Survey here

For contact info, we can ask Yoon but i think it should be addressed to him with attn: HPSA at the school mailing address. instead of giving a contact phone # lets do an email. maybe Gillian's since she is the youth health chair. (if you agree)

Here is my input. (the number indicates the question # on questionnaire)
1. Add awareness of healthy behaviors, safety/security as options
2. These health problems seem geared to adults. For kids the main ones I can think of are: overweight/obesity, dental, poor nutrition, environmental/unsafe atmosphere, school violence, mental health, other
3. Change shots to immunizations. Add drug abuse (illicit or over the counter), alcohol use, poor self-esteem

I think we should add a question along these lines: Please indicate any specific programs/events you would like HPSA to foster at your school: Run/walk clubs, After school activities, Nutrition series, Mentor/self-esteem group, Tobacco awareness seminar, Recycling, Other.
Under that we should say: If interested please leave a contact name/number for planning.
Those that respond obviously are interested in our help and we can plan to help them.

I like all three comments made. I think that Dr. Seol should be the contact person. I also agree with keeping the contact info via email is smart. From the "real age" questionaire we did at the last meeting, i think we may be able to pull questions from that for this survey. I will take a look and bring what i have to the next meeting.
